Insurance and Payments
We will file your insurance claim on your behalf. However, in order for us to obtain payment, all of your health insurance information such as coverage, benefits, co-payments and referrals must be accurate, current and presented to our staff at the time of your appointment. Ultimately, payment for medical services is your responsibility.
